Registration
Date and Times
Registration for all students of Peru Public Schools District
124 will be held on Tuesday, August 18, 2009, from 10:00 a.m. to
7:00 p.m. at Parkside School. Parkside is located at 1800 Church
Street, at the corner of Church Street and Shooting Park
Road.
IMPORTANT –
Required Identification to Register
All parents and
guardians are REQUIRED to provide TWO FORMS of evidence of CURRENT
RESIDENCY within the boundaries of the school district.
Proof of residency must include a driver’s license or other
state-issued ID, as well as a second, recent form of identification
with your name and address. This may include a recently dated rent
receipt or current utility bill. Registration
will not be allowed without absolute proof of residence, regardless
of past attendance in the district. For more details on

IMPORTANT – Immunization and Physical/Dental Exam
Requirements
All students are required by law to have updated immunization
records. Students entering kindergarten or sixth grade must provide
evidence of a physical exam that has been completed within one year
prior to the first day of school. All
students new to the district must also show proof of a physical
exam. All physical exams are due by the FIRST DAY of student
attendance. Students in kindergarten, second and sixth grades must
provide evidence of a completed dental exam. Students entering
kindergarten must provide evidence of a vision exam performed by a
licensed Illinois optometrist or ophthalmologist. This is also due on the
first day of school.
Birth Certificates
All
students enrolling in the district for the first time are required
to present a certified birth certificate, which can be obtained
from the county in which the child was born.
Optional and Required Fees
Registration fees include a book rental of $50.00 for all K-8
students. Separate fees will be collected from Parkside students
for physical education, as well as students who will be
participating in band and chorus activities. No fees are charged
for preschool. More detailed information will be available at
registration.
Breakfast and Lunch
Student breakfast is $1.25 per meal and student lunch is $2.00
per meal. All students must purchase a lunch card with a beginning
minimum balance of $10.00 to participate. Reduced price information
based upon family income eligibility guidelines will be available
during registration.
IESA Sports Physicals
Students participating in any athletic activities throughout
the school year must have a completed physical examination prior to
the first day of practice for their season. For those interested,
representatives from Illinois Valley Community Hospital will be
present at Parkside on Tuesday, August 18, from 10:00 a.m. – 7:00
p.m. to administer IESA sports physical examinations, which will be
effective for one calendar year. The cost for these physicals is
$20.00 per student.
Transportation
Students who reside more than 1½ miles from the school in which
they attend, or who live in an area approved as a hazardous zone by
the Illinois Department of Transportation, will be provided free
bus transportation to and from school. More information will be
available at registration.
2009-10 Calendar and
Opening Schedule
We are pleased to announce that the 2009-10 school year will
begin on time as planned. The first official day for students at
their new schools will be Wednesday, September 2 with a noon
dismissal, followed by 2:00 p.m. dismissals from September 3
